The following items are required to register a new student at Los Alisos Middle School.
- Proof of residency (gas, electric, water bill, escrow paper or rental aggreement)
- Immunization records
- Check Out papers from previous school with school name and address
- Copy of student's recent report card
- Birth Certificate
- Copy of IEP if student is in an RSP/SDC/Speech Special Education Program
- If you are residing with a friend or family member and have no proof of residency the following items are required:
- You need proof of residency form notarized
- Electric, gas, or water bill with the name and address of the person you will be residing with
- Legal documents of Custody placement if applicable
- If it is necessary for your child to take medication at school, please ask secretaries for a medication form. This form needs to be filled out by Doctor prior to taking medication at school.
